Role Scope :

We are looking for an experienced Clinical and Corporate Trainer who is passionate about preparing PACE Center team members to provide the best care possible to our unique participant population.

  • Support PACE Center Openings : support successful launch of additional Habitat Health PACE sites by supporting on-site training and education to team members (clinical and non-clinical)
  • Support PACE Center Openings : train on-site leaders on how to conduct role-specific training and playbook reviews for new hires, and ensure they're ready to train new hires once your deployment to that PACE Center has ended
  • Support PACE Center intensive training needs : facilitate additional on-site training when additional support is needed (beyond what is required for successful Center openings) due to leadership vacancy, high-volume of retraining needs, or high-volume of new hires
  • Develop materials for clinical orientation for existing staff when new clinical programs are rolled out
  • Develop and evaluate ongoing continuing education programs aligned with organizational goals and participant needs.
  • Identify clinical skills and topic areas where additional training is needed, in partnership with the CMO, Practice Operations, Quality teams and local leadership, and create and deliver education and training for those areas.
  • Serve as a subject matter expert on best practices in gerontological nursing and chronic disease care.
  • Incorporate adult learning principles and innovative delivery methods (e.g., e-learning, simulation).

Qualifications:

  • Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing, Education, or a related healthcare field preferred.
  • Current RN license in the state of practice (active and in good standing).
  • BLS certification
  • Minimum 3–5 years of clinical nursing experience, preferably working in the field of PACE directly; experience in geriatrics, long-term care, or home health otherwise required.
  • Minimum of 3 years experience providing on-site, role-specific training
  • Ability to travel up to 75% of the time
  • At least 1–2 years of experience in clinical education, staff development, or training.
  • At least 3 years experience working with frail elderly populations or individuals with complex medical needs.
  • Strong understanding of adult learning principles and evidence-based teaching strategies.
  • Ability to assess learning needs and develop competency-based training programs.
  • Excellent communication, interpersonal, and organizational skills.
  • Culturally competent and sensitive to the needs of diverse populations.

  • Ability to lead with influence

Nice to have:

  • Certification in Nursing Professional Development (e.g., ANCC NPD-BC) or Gerontological Nursing (e.g., RN-BC).
  • Experience coordinating orientation and onboarding programs for clinical staff.
  • Knowledge of quality improvement processes and data-driven training interventions.
  • Familiarity with PACE regulatory requirements (e.g., CMS, state-specific guidelines).
  • Proficient in electronic health records (EHR) and common office software (e.g., Microsoft Office Suite).
  • Experience with eLearning Platforms such as Rise/Articulate.

Compensation:

We take into account an individual's qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $83,800 - $112,000 and is bonus eligible. The actual offer will be at the company's sole discretion and determined by relevant business considerations, including the final candidate's qualifications, years of experience, skillset, and geographic location.
